Source notes
- The specification questionnaire (pp. 58–59) refers to Groups 1 through 5 on an all-or-none basis and pricing all items within a group, but the pricing proposal (pp. 60–62) contains four named tables and labels the emergency response fee optional (p. 61). The pricing/group instructions are inconsistent.
- The specification questionnaire (p. 57) says to complete and upload References, while References.pdf (p. 1) requires three references to return the signed two-page form directly by email to wsdprocurement@phoenix.gov by November 6, 2026, 2:00 p.m. Phoenix local time. Bidder upload alone should not be assumed to satisfy the direct-reference instruction.
